Organes sexuels externes féminins avec des malformations de formes très diverses: phénotype d’un étalon jusqu’au phénotype d’une jument avec des signes de virilisation et d’intersexualité: p. ex clitoris développé comme un phallus (clitoris trop gros ou pénis trop petit); distance anogénitale anormale; hypospadias (malformation congénitale du développement de l’urètre, ouverture dorsale); vagin en cul-de-sac, absence ou hypoplasie des organes génitaux féminins internes (vagin, col de l’utérus, utérus); gonades intra-abdominales: petits testicules sans spermatogénèse (mâle XX et cryptorchidie) ou intersexualité (présence simultanée de tissus ovariens et testiculaires: hermaphrodisme vrai, hermaphrodisme XX); comportement d’étalon. Restriction grave d’une fonction organique (organes sexuels); la qualité de la vie peut être affectée; troubles du comportement qui peuvent rendre difficile la cohabitation avec des congénères; les animaux atteints ne peuvent parfois pas être détenus conformément aux besoins de l’espèce; risque d’automutilation; risque de frustration et de mauvais traitements si le cheval est puni injustement. La castration est rendue plus difficile et plus coûteuse. Les critères déterminants pour l’évaluation des contraintes sont en particulier:
